SHS OFFICIAL SCHOOL FORM 9 | Three-Term Ready (SY 2026-2027)

The Ready-to-Use School Form 9 (SF9) for Grades 11 and 12 for School Year 2026–2027 is now available for Senior High School teachers and class advisers. The file is prepared for the three-term grading system and may be used to organize and prepare learners’ progress report cards more efficiently.

The SF9 provides the necessary sections for recording learners’ grades, General Average, attendance, remarks, parent or guardian signatures, and other required learner information. Teachers are encouraged to carefully verify all grades, attendance records, and learner details before printing and releasing the report cards.
