SCHOOL FORM LABELS, COVERS (Editable and Free to Download)

A streamlined set of school forms was introduced and implemented through DepEd Order No. 4, s. 2014, titled "Adoption of the Modified School Forms (SFs) for Public Elementary and Secondary Schools Effective End of School Year 2013–2014." Currently, SFs 1, 2, 4, and 6 are used to record learner data from Kindergarten (excluding SF 5) through Grades 1 to 10. SF 3 is designated for documenting textbook distribution, while SF 7 captures information regarding the current official duties or teaching assignments of school personnel. These modified forms serve as a vital source of data for evidence-based assessment, planning, resource allocation, performance monitoring, and evaluation.
