The Department of Education (DepEd) has released the revised School Form 10 (SF10) for the End of School Year (EOSY) 2025–2026. This updated form is designed to improve the documentation of learner progress, academic performance, and other essential information needed for student transfers and effective school administration under the MATATAG Curriculum.
Commonly known as the Learner’s Permanent Academic Record, the SF10 provides a comprehensive record of a student’s educational journey from Grade 1 to Grade 12.
